Co-Op and Intern Requirements
The requirements for the cooperative education program and the
industrial internship program are the same:
- Contact Information -- send us your address and phone number as
soon as possible each term
- Goal Statement -- work with your supervisor on this, then submit
within the first two weeks of your work assignment
- Final Report -- submitted when you get back on campus, but make
sure your supervisor has approved it
- Performance Evaluation Form -- by your supervisor, submitted with
your final report or mailed by your supervisor
- Technical Elective Credit Justification Form (optional, this
information can be included in your final report)
Required forms are available here. No
form is required for your goal statement or final report. They can be mailed or emailed to
the addresses below, or faxed to 406-994-5308.
