Personnel Data Update Form - Logon Page

There are two levels of security associated with this form:

1. Windows NT password protection - only MSU CHE students may access the form.
2. Personal password - your data will not be available to other students.

When you go to the Data Update Form, you will be asked to enter your username and password (once per browser session.)

• If you have never entered data before, you must register before you can access the form.
• Always use the same username and password when you update your data.
• Be sure to close your browser when you are done - otherwise the next user could access your data.
